The type of personal information I will collect

Your name, address, telephone number, email address, next of kin details, date of birth, GP details, medical history/details (including current medication)

 

Why am I asking for the information?

I’m not being nosy.  I need to ask about certain things, like your medical history, as it affects your feet.  I want to give you the best possible treatment when you visit and to do this I need you to be honest about your health and lifestyle.  I am required by law to keep records of your treatments and this information forms part of this.  If you register your mobile phone or email number I may send messages as reminders.

 

Sometimes it’s necessary to share some if your information.  So who will we share it with?

Primarily with you and where necessary or required we also share information with other healthcare providers (such as your GP), your welfare guardian or carer (with your consent). 

 

When I do have to share your data, I am required to comply with all aspects of the Data Protection Act (DPA), the UK General Data Protection Regulation(UK GDPR) and are registered with the Information Controllers Office (ICO)

 

AT NO POINT WILL ANY OF YOUR INFORMATION BE PASSED ON TO ANY OTHER THIRD PARTY

 

How will the Data be stored?

Some of the records are now being held digitally and are stored in line with the law.  Some of the data is stored outside of the European Economic Area (EEA) and these servers are all compliant with the relevant data protection laws.

 

How long will the Data be stored?

In line with the legal requirements of holding medical data, your record is stored for 7 years (or in the case of a minor until that minor reaches the age of 25 years) from the date of last treatment by us.

Please note that your right to be forgotten cannot override the legal requirement to keep medical notes for the mandatory period

 

When will the Data be destroyed?

All data will be destroyed after 7 years, or in the case of a minor once that minor reaches the age of 25 years
